A trap is a hidden danger that can cause sudden unwanted conditions and/or damage. A vent that spews toxic gas or a that zaps creatures that trip it are both examples of traps.
The perception skill is used to detect traps and a trap disarm toolkit along with the sleight of hand skill is used to disarm traps. Vents can also be "disarmed" by placing a heavy enough object on-top of them.
Some traps will be visible without a Perception check, but unless a character detects them they cannot be disarmed and can only be avoided.
