Mind Flayer Tadpoles are Aberrations used by the Illithid to transform humanoids into Illithids through the process of Ceremorphosis. The player character and origin companions of Baldur's Gate 3 are infected with tadpoles. One of the first main Quests is to Find a Cure Find a Cure.

It is revealed in Act One that the Cult of the Absolute is related to the Illithid abductions. By seeking out different healers to find a cure, the party learn more about their affliction. It is also revealed that the player character can absorb more tadpoles, unlocking Illithid Powers.
