- These Ghouls have the Condition Gnawing Horde, but what, if anything, it does is unknown.
- These Ghouls have a variation of Death Burst which deals 6d6Necrotic with a Dexterity saving throw for half. This affects allies and enemies alike, and can cause a chain reaction with other Ghouls. This does not have a name or icon or appear under Notable Features.
- Ghouls are immune to . This prevents them from being driven due to a failed Wisdom saving throw when the summoner wears Abyss Beckoners.
