Headless Ghost is an NPC that appears when Sarin's Skull is looted. It will follow the party while located in Lower City Sewers and have the skull in a party member's inventory.

Leaving the sewers or sending the skull to camp will make the ghost disappear, but it will reappear when the party returns to the sewers while carrying the skull.

Headless Ghost can not be interacted with and there is nothing to pickpocket.

Returning Sarin's skull

Spoiler warning! This section reveals details about the story of Baldur's Gate 3.

After returning the skull to Sarin's skeleton, the ghost will appear whole with Sarin's head, bow in thanks, and disappear. The player will receive Magic Amulet as a reward. If you do this in turn based mode, you can kill Sarin's ghost and obtain xp but you will not receive the amulet and there is no available loot on the body.
