Found displayed behind Zevlor's corpse in the party camp. This item can only appear at the start Act 3 after a long rest, provided that the player follows specific actions through the game:

• The tieflings must be saved in Act 1, and ensure Zevlor survived.
• The player must find and interact with the pod containing Zevlor in the Mind Flayer Colony in Act 2, but choose to not free him.
