Soul Coin is a Story Item that can be found in various places in Act 1.

Strange, incomprehensible whispers emanate from this coin, pervading your mind with rage and despair.

This is a list of confirmed Soul Coin locations:

Dank Crypt, inside one of the sarcophagi on the western side of the heavily trapped room.
Dank Crypt, inside a sarcophagus in the small room west of the shrine to Jergal, where the Book of Dead Gods can also be found.
Nadira, after saving her from a Bugbear assassin, above the entrance to the Druid Grove.
