To gain the Permanent Bonus, Loviatar's Love Loviatar's Love, you will need to be hit three times, and pass each subsequent constitution/performance/intimidation rolls while staying above 40% hp.
I was able to get the blessing with Karlach with failing one performance check. I went from intimidation (passed with critical success) to constitution (failed) to performance (passed). And was blessed in the end. A second time I tried the same order and failed critically on the second throw and still got the blessing.
