A Full-Illithid is a character who has undergone complete ceremorphosis and become a Mind Flayer. This option will only become available after travelling to the Astral Plane during the quest Confront the Elder Brain.

In order to transform, the player must offer to do so while in dialogue with The Emperor or Orpheus once they have reached the Astral Plane after travelling to the Morphic Pool. If Karlach was brought along, she will also offer to transform.
