Death's Head of Bhaal Feona is a member of the Cult of Bhaal, found in the High Security Vaults in The Counting House in the Lower City.
Involvement
During the 
quest, the party can go to the Counting House in hopes of catching up to Minsc. Inside the vaults, the party witnesses Minsc emerge from a Mimic and defeat it. 'Jaheira' then appears with a handful of Bhaalists including Feona, and teleports away with Minsc, leaving the cultists to attack the party and the staff.
